Over The Counter (OTC) Sales

INTRODUCTION
According to the Food and Drug Administration (FDA) regulations, a drug must be safe and effective to be sold over the counter. An OTC drug is safe when it has low incidence of adverse reactions and side effects under conditions of widespread availability. An OTC drug is effective, if there is a reasonable expectation that when used under adequate direction for use, which warnings will provide significant relief for the ailment for which it is being taken to a significant portion of the targeted population.

SIGNIFICANCE OF OTC COUNTERS
For most consumers, information on non-prescription drugs comes from the label put on the product. FDA requires that OTC labels should be more detailed as compared to prescription drug labels. The label on which non-prescription medicine should include the following information:
Product name
Active ingredients
1. Inactive ingredients
2. Name and address of the manufacturer
3. Net quantity of ingredients
4. Description of tamper resistant features
5. Indication for use
6. Directions and dosage instructions
7. Warning and contra-indications
8. Precautions on drug interactions
9. Over the Counter (OTC) Sales
10. Expiry date
11. Batch number

SALE AND ESTABLISHMENT OF OTC MEDICATION
1. This means establishment of a medical store, which performs the functions of dispensing, selling and distribution of non-prescription and prescription drugs. The various factors which have to be considered before establishing a drug store are:

2. Acquisition of Premises: The premises may be purchased or rented on lease for a certain period of time. The cost of premises depends upon the location of the premises.

3. Location of Premises: Generally, the premises should be located on the ground floor of the building. The outer front door of the drug store should be very attractive and build with innovative construction techniques.

4. Layout of the premises: The primary function of an OTC counter is to provide every possible convenience and aid to the customers to make their shopping easy and less tiring.

5. Equipment and facilities: The equipment in the pharmacy should be suitable and adequate for the work to be undertaken. All equipments must be maintained in an accurate working condition.

RATIONAL USE OF COMMON OTC MEDICATIONS:
Rational Use of Drugs (RUD) is at the core of pharmaceutical best practice. Rational use of medicines refers to the correct, proper and appropriate use of medicines. Rational use requires that patients receive the appropriate medicine, in the proper dose, for an adequate period of time and at the lowest cost in addition to this use of an appropriate, efficacious, safe and cost effective drug given for the right indication in the right dose and formulation, at right intervals and for the right duration of time.
Factor Contribute to Incorrect Use of Medicines
1. Lack of skills and knowledge.
2. Inappropriate unethical promotion of medicines by pharmaceutical companies
3. Profit from selling medicine.
4. Unrestricted availability of medicines.
5. Overworked health personnel.
6. Unaffordable medicines.

Examples of OTC Drugs:
1. Topical anti-bacterial, anti-fungal OTC drugs: Bacitracin, Clotrimazole (vaginal, topical use), Miconazole 2% ointment, etc.
2. Pain OTC drugs: Acetaminophen, Aspirin, NSAIDs, Ibuprofen, Naproxen, etc.
3. Smoking cessation OTC drugs: Nicotine patch.
4. Topical dermatological (skin, scalp) OTC drugs: Capsaicin, Doak tar distillate oil, Hydrocortisone, Permethrin, Pyrethrin, Zinc oxide ointment.
