One of the most prevalent types of primary headaches is the tension-type headache (TTH). Tension headaches are often described as a feeling of a tight band or pressure around the forehead, with dull, aching pain that can extend to the sides or back of the head. Here's an in-depth look at tension headaches, their causes, and how to manage them.

What Are Tension-Type Headaches?

Tension-type headaches are the most common form of primary headache, affecting up to 80% of the population at some point in their lives. These headaches can be episodic (occurring less than 15 days a month) or chronic (occurring 15 or more days a month for at least three months). They are generally less severe than migraines but can still be debilitating, especially when chronic.


Symptoms of Tension-Type Headaches

Tension-type headaches typically present with the following symptoms:
A constant, dull, aching pain is usually described as a band-like tightness around the head.
Pain that is often bilateral, affecting both sides of the head.
Tenderness in the scalp, neck, and shoulder muscles.
No associated symptoms like nausea, vomiting, or aura (which are familiar with migraines).
Sensitivity to light or noise may be present, but it's generally milder than in migraines.
Causes and Triggers
The exact cause of tension-type headaches is not fully understood, but several factors are believed to contribute to their development:
Muscle Tension: Prolonged muscle tension in the neck and shoulders, often due to poor posture, stress, or anxiety, is a significant contributor.
Stress: Emotional or mental stress is a well-known trigger for tension-type headaches. Stress can cause muscle tightening and trigger headaches.
Sleep Issues: Insomnia or disrupted sleep patterns can lead to headaches, creating a cycle where headaches further disrupt sleep.
Dehydration and Hunger: Skipping meals or not drinking enough water can trigger headaches.
Eye Strain: Spending long hours in front of screens or straining the eyes can contribute to the development of tension headaches.


The Role of Geography and Gender

Interestingly, the prevalence and impact of tension-type headaches can vary based on geographic location and gender. Studies have shown that women are more likely to suffer from tension headaches than men, with ratios varying by region. For example, in Turkey, the female-to-male ratio for tension headaches can be as high as 6:1, while in North America, it tends to be closer to 2:1​


Diagnosis and Treatment

Diagnosis: Tension-type headaches are diagnosed primarily based on the patient's history and symptom description. There is no specific test for diagnosing tension headaches, but doctors may perform examinations to rule out other causes of head pain, such as migraines or secondary headaches.


Treatment:

Lifestyle Modifications: Managing stress, improving posture, staying hydrated, and ensuring regular sleep can help reduce the frequency of tension headaches.

Over-the-counter Medications: NSAIDs (like ibuprofen) or acetaminophen are commonly used to alleviate the pain associated with tension headaches.

Prescription Medications: For chronic tension headaches, doctors may prescribe tricyclic antidepressants or muscle relaxants to reduce the frequency and severity of headaches.

Cognitive Behavioral Therapy (CBT): CBT can be effective in managing tension headaches by helping individuals cope with stress and change negative thought patterns that contribute to the pain.

Physical Therapy: Techniques like massage, stretching, and exercises to strengthen neck and shoulder muscles can alleviate tension.

Relaxation Techniques: Meditation, yoga, and other relaxation practices can help manage stress and prevent headaches.
